Famous faces entering this year's I'm A Celebrity must reveal how much alcohol they drink on a weekly basis and their regular medication in order to prevent any meltdowns in camp.   

ITV bosses are only able to 'make judgements' on whether or not a contestant is suitable 'when they have the forms in front of them'.

The intimate questioning is said to have been put in place in order to 'avoid Gemma Collins style meltdowns'.

A source told The Sun: 'The questionnaires are very detailed and for good reason. The producers need to know if a celebrity is physically and mentally fit enough for the challenge ahead.

'It is, after all, an entertainment show and there's no point signing up someone who won't cut the mustard because they can't perform any of the tasks.'

The paper added that campmates must also 'provide details of any medical conditions like sleep walking, night terrors, phobias and vertigo as well as permission for the show's medical team to contact their GPs'.
