AFLW star Moana Hope all but confirmed her split with her wife Isabella Carlstrom last month when she removed any reference to her in her Instagram bio.

And now Carlstrom has officially revealed the pair have parted ways.

The model made the announcement on her own Instagram page on Monday morning, and said they were following in the footsteps of Gwyneth Paltrow and Chris Martin by 'consciously uncoupling'.

The term refers to an 'amicable split' and was made popular by the Hollywood star and Coldplay frontman back in 2014 when they announced their shock breakup.

'It's with a heavy heart that Mo and I have decided to separate. We will continue to raise out beautiful kids together as we consciously uncouple. We appreciate all the love and support over the years, thank you,' she wrote in her statement.

The mother of two is an ambassador for Cocobella and took part in the Mother's Day Classic Fun Run the at the Alexandra Gardens over the weekend, in honour of her mother-in-law who is a survivor of breast cancer.

'My mother-in-law, Belle's mum, she is a survivor of breast cancer and somebody I admire and love very much, so to know I have someone close to me that has been affected, that I can potentially run for, means more than you can possibly imagine,' Hope told the Herald Sun.

The Survivor star pushed her son Ahi, 10 months, in the pram during the run.

She went on to say she was focusing on her children amid her heartbreaking breakup with Carlstrom.
