Paul Newman, 42, who had already been jailed for breaking the jaw of partner Geraldine, 51, battered her with a hammer. He stabbed Shannon, 11, and Shane, six, before he hid their bodies under a duvet covered with rosary beads and toys. Newman then travelled 140 miles from the family home of Allerton Bywater, near Leeds, to jump from a cliff at Holyhead, where he had proposed to Geraldine 17 years earlier.
Retired detective chief superintendent Paul Johnston, who carried out a review into the 25 organisations involved with the family over several years, said what happened could not have been foreseen by "friends, family or professionals".
Wakefield coroner's court heard that the couple had reunited after Newman was jailed in 2013 for two years for the horrific assault.
But the marriage was said to be "deteriorating" at the time of the tragedy in February 2016.
